Case management for supported employees with very low productivity

The Community Grants Hub, on behalf of the Department of Social Services (DSS), is inviting providers via a restricted process to apply to deliver services under the Disability, Mental Health and Carers Program – Disability and Carer Service Improvement and Sector Support – Case management for supported employees with very low productivity for 2016-2017.

The objective of this Grant for Case Management is to provide an opportunity for supported employees in Australian Disability Enterprises (ADEs), who have very low productivity, to make informed choices about their future.

The Grant will fund eligible ADEs to engage individual case management services for participants. Case Managers will help participants explore alternatives to employment in an ADE by helping them learn about, consider, trial and choose other day time activities.

DSS recognises that not all participants will choose to leave supported employment. Case management services will also include planning materials for participants that decide to stay in supported employment for the time being, to assist with any future decisions about leaving supported employment in an ADE.
